Italian authorities had already said Messina Denaro was trapped because he was in poor health, but on Friday they revealed the source of a key tip.

Police assumed the note belonged to a relative and, after confirming that no family members had similar cancer problems, focused on Matteo.

The mobster was convicted of a long list of crimes, including the murder of top prosecutors Giovanni Falcone and Paolo Borsellino and a teenage boy whose body was dissolved in acid.

While on the run, Messina Denaro followed the Mafia tradition of communicating with relatives and affiliates via “pizzini,” small pieces of paper sometimes written in code.
According to the police, his sister was one of the operators of the “pizzas” but did not follow his brother’s advice to burn them after use.
They claim he had a major role as a treasurer in the mafia family and say he communicated with Messina Denaro using the code name “Fragolone” or “big strawberry”.
Her home is decorated with a large portrait of her fugitive brother, with a police video showing him wearing sunglasses and a crown.
Rosalia’s husband, Filippo Guttadauro, is another convicted Sicilian mobster, while her daughter, Lorenza, is Matteo Messina Denaro’s protégé.
